Ruby Gettinger, a lovable 487-pound woman from Savannah, Georgia, who has struggled with obesity her entire life. Ruby lives in a small two-bedroom house with her dogs, her nephew Jim and old friend Jeff. Together they share everything from laughs to a love of fattening Southern foods! But now Ruby's pals are realizing that they need to give her tough love in order to help her slim down.

After three months on a rigorous diet and exercise plan, Ruby is finally starting to see results and has already shed fifty pounds! Word of her weight loss has spread among friends and Ruby has recently been getting phone calls from her ex-boyfriend Denny who broke her heart six years ago when she was still struggling to lose weight.

Thanks to four months of strict diet and exercise, Ruby has lost 60 pounds! But she's growing sick and frustrated with her pre-packaged meals and she's feeling tempted to cheat on her diet, especially when she goes out to restaurants with her friends who gorge on pizza and other fattening foods right in front of her.

Ruby has become completely frustrated with her life, her diet and her training. She's hit a wall and is annoyed by everyone and everything! Although she has lost 60 pounds in almost four months, Ruby reveals she can't tell the difference in the mirror and it's making her unmotivated to continue losing weight.

California, here we come! Returning to Los Angeles where she lived for six years, Ruby must endure the challenges of travel for an obese person. Although she has lost over 65 pounds, she still has trouble fitting in the airplane seats and must put up with public humiliation in front of her friends.

Upon returning to Savannah from her trip to Los Angeles, Ruby is shocked to discover that she had gained back some of the seventy pounds she had lost over the last five months.

It's been one year since Ruby started her weight loss journey and she's managed to shed more than 100 pounds. Now at 361 pounds, Ruby is one step closer to dipping under 350 pounds for the first time in her adult life.

Ruby is determined to dip under 350 pounds, and she knows that she needs to ramp up her exercise program if she's ever going to reach her goal weight. However, lately she has been feeling unmotivated by her trainer Reese and his monotonous workout plan. Ruby really wants to try more creative ways to work out, so she reaches out to Dr. Bradley who recommends she meet with Dr. Jackowski, a fitness guru.

Now that she is one hundred pounds lighter, Ruby is determined to reach her goals and she is eager to get back into the real world and find a part-time job for the first time in 10 years.

Ruby's weight-loss woes continue as she searches for a new trainer to help her reach a milestone weight of 350 pounds. But what will it take to help achieve what seems like an impossible task?

During a pilates class, Ruby becomes emotionally overwhelmed when a flashback is triggered from her childhood. Her instructor explains that the mind and body share a powerful connection and old memories that are stored away can become activated unexpectedly.

Ruby turns to a new therapist to help understand her recurring nightmares.

Ruby goes to Mississippi for a friend's birthday, but takes a detour to explore the missing memories of her childhood.

Ruby visits her ex Denny in Myrtle Beach for the weekend and wears a swimsuit for the first time.

Ruby encourages Christina to take part in the Plus Teen USA pageant and helps with her transformation.

Ruby spends a night in a sleep clinic and tracks down Dr. Dukes who helps shed light on her missing memories.

As Ruby struggles with losing weight, her therapist suggests she give up five unhealthy things from her life.

Ruby delves into her man issues and a tense encounter with her ex-boyfriend Denny leads to a couples counseling session with Dr. Jane.

Following her weight gain, Ruby discovers letters from her childhood while cleaning out the attic and is shocked and upset at them. She has a counseling session and a meeting for her Woman's Fat Night group, where pastor Paula White makes an appearance.

Ruby & friends have a garage sale to raise money for Ben's move to Los Angeles for music school. Ruby's childhood friends show up and they have a dinner. Meanwhile, Ruby tries to discover more about her letters with the help of pastor Paula White & her therapist. Ruby & friends then head out to a horse center where they get Zach, who is autistic, to ride a horse in hopes of him forgetting that Ben is leaving.

Ruby & the gang go out to LA to help Ben set up for music school. While there, they enjoy time at a water park. Ruby then speaks with Melissa and they go to a Reiki session, where she starts blowing up with emotions. They then get Ruby into the backyard pool where she overcomes the fears of the deep end.

Ruby, Georgia & Jeff go to a mall, where she meets someone at Lane Bryant. She then decides to go to a yogurt shop, while her friends go get hot dogs on a stick, which is a decision she didn't like. The new friends agreed. Ruby sees what Jeff is talking about when he says that she is sick. Ruby has a Woman's Fat Night with her new LA friends. She endures a beach workout with a trainer that she thinks is trying to hurt her. During the episode, she weighs in, only to discover that she has gained even more weight.

Ruby & friends go to a show, where she meets a guy named Ian. All throughout the episode, Jeff & Georgia think that Ian & Ruby are in a relationship, something she constantly denies. She goes for a walk on a long path, having breathing troubles, but ends up making it to the end. She then goes to Ken Paves and gets hair extensions. She invites Ian to Ben's concert, then they go to a dream art gallery. They then finish off at a French restaurant. The next day, Ruby has a discussion with her close friends about the date the previous night. The episode closes with a conversation over the phone with Dr. Jane.
